When shooting videos they often have photographers taking pics during the shoot, often hundreds of pics are made. Most likely the scene was dropped from the video but used (inadvertently) on the cover. Sloppy ... but porn studios aren't known to be sticklers about detail.

I don't recognize any of the guys, either.

Ah, I found him. His name is Wayne Montgomery, and the movie that the scene is from is Dirty Pillow Talk.

I know how you feel about wanting to find a certain video. Years ago I downloaded pics of several scenes from "Hollywood Hunks" and I wanted to see the video but I didn't have its title. I didn't search for it until I stumbled upon a pic and the name of one of the "actors," T.J. Scott. From that clue I found the video. A rather good one, BTW.
