It's my impression that poison sumac is chemically very similar to poison oak, with which I am very familiar.
With poison oak, it is not spread by rubbing. The areas that get the rash are those directly exposed. Think of it as a chemical burn. EXCEPT, when the irritating chemical gets into the blood stream. A way this happens is if you are scratched by a poison oak branch or twig. In these cases, the rash can start popping up all over the body. This is referred to as systemic poison oak.
I had it last year for six weeks and finally went to a walk-in clinic where they gave me a shot of prednizone. It basically cleared up in a couple of days. I'd at least look into that option as it sounds systemic to me. They can also give you a five day step-down of prednizone pills but I don't think it works as well and, to me, is a hassle.
