That's just a small segment from his scene, which starts with Karel, as a soldier, going for a check-up at the docs. He goes behind a screen and strips off, putting on a short medical gown, far too short to cover his monster cock which hangs out under the gown.
The doc and his assistant then 'examine' Karel in intimate detail, including sitting him in the gynaecology exam chair, which gives total access to Karel's genitals.
