They are reffering to a plastic cylinder which is placed over the penis, then the air is puped out with either a manual or electric vacuum source. This causes the penis to swell in size due to the internal pressure inside the penis being greater than the outside pressure. Most of the expansion is due to fluid from the body filling the expanded tissue. If done correctly, it is not harmful, correctly meaning not using extreme vacuum levels, and taking frequent breaks to allow fresh oxygenated blood into the tissue. It provided temporary enlargement, especially thickness, and with very long term repeated use, many have experienced permanent enlargment as well, but everybodies experiences vary.
